Originator's family name, Initial(s) year, Title, map, scale, Series, sheet number, Publisher, Place of Publication, 'Cited location title', coordinates.

Notes:

Originator can be the cartographer, surveyor, or compiler
Include Series and/or sheet number only if applicable
Include Cited location title and coordinates only if citing a specific location on the map
